Authority to Operate

1.(a) ccCNA is REGISTERED UNDER THE UTAH POSTSECONDARY PROPRIETARY SCHOOL ACT (Title 13, Chapter 34, Utah Code).  (b) Registration under the Utah Postsecondary Proprietary School Act does not mean that the State of Utah supervises, recommends, nor accredits the institution. It is the student's responsibility to determine whether credits, degrees, or certificates from the institution will transfer to other institutions or meet employers' training requirements. This may be done by calling the prospective school or employer. (c) The institution is not accredited by a regional or national accrediting agency recognized by the United States Department of Education. Our Facility

2. The ccCNA classroom is fully equipped with hospital beds, IV poles, mannequins, desks, chairs, multi media projection equipment, built in sound system, wheelchairs, catheter bags, crutches, walkers, bedside commodes and the equipment and supplies needed for hands on learning. Phlebotomy supplies include mannequin arms with simulated blood return.

Owners and Instructors

3. Claudia Wilcox, owner, has been a Registered Nurse for 30+ years. Claudia has taught CNA classes for 8+ years. She has worked as a Registered Nurse in long term care, emergency, pediatrics, home care, cardiology, endocscopy and OR.

4. Carol Lindsay, owner has been a Registered Nurse for 15+ years. Carol has taught CNA classes for 12+ years. She has worked in long term care, pediatrics, emergency, and labor & delivery. She holds a BSN in Nursing and a Masters in Management. She is currently enrolled in a Masters in Nursing Education program at the University of Wyoming. In addition to teaching Carol has been published in NurseWeek, Nursing Spectrum, American Journal of Nursing, Journal of Emergency Nursing and Nursing.

Courses Offered

6. Nurse Aide Training - (a) ccCNA offers an 80 hour training course that prepares students to apply for and complete the state testing allowing them to be certified by the state of Utah as certified nurse aides and be placed on the state registry. (b) At completion of the class students who pass our test and complete clinical hours will be placed on the state “completer” list and be given an application stating they have completed the requirements set by the state to take the state nurse aide certification test. (c) Classes run throughout the year on a biweekly or monthly basis.

7. Phlebotomy Basics - (a) ccCna offers a Phlebotomy Basics class. (b) This class introduces students to the concepts and skills of phlebotomy that will be used if hired as a nurse aide in a hospital setting. Additionally, it will permit students to list phlebotomy class completed on resumes and applications to nursing school. There is no state certification or registration for phlebotomy. Students will receive a certificate of completion for the class. Students will not complete clinical hours in phlebotomy outside of the six hour classroom time. (c) Classes are six hours in length and will be offered a minimum of once per month through out the year.

Cost

8. Total costs and payment plans for each program, including application, registration fees, tuition, books, supplies, equipment, laboratory, special tutoring, late payment, and graduation fees. (a) There will be no collection of prepaid or unearned tuition or fees.  (b) There are no payment plans. (i) full tuition to be paid on the first day of class. (ii) an initial deposit of $50 is requested 2 weeks before class begins to hold place in class. The $50 is non-refundable past the 3 day cooling off period. The $350 if paid in advance is refundable in full if the student chooses not to attend the class. Total cost for the C.N.A. class is $400. Total Cost for the phlebotomy program is $150. (c )There are no installment plans and no interest.

Admission Requirements

9. (a) Student admission and enrollment requirements (minimum is possession of a high school diploma, General Education Development (GED) Certificate, and being beyond the age of compulsory high school attendance, as prescribed by Utah law. (See Rule 152-34-4(3) of the Utah Administrative Code.)  (b) There is no granting of prior credit such as reduced hours in the program or any tuition discount for any previous education or training completed.  (c) Students in the nurse aide course must attend all 80 hours of nurse aide class to complete the class. Students in the phlebotomy class must attend all 6 hours of phlebotomy to complete the class. Classes may be made up within a 6 month period at not additional charge. To pass the C.N.A. class and qualify to take the state test student must pass a final exam with 83% (135/150 test questions) Those who do not pass the C.N.A class may retake the class at no additional charge up to three times in a 6 month period after the original class is completed.(d) Students who practice inappropriate behavior in the classroom or clinical setting will be expelled from the program. This includes: drugs, alcohol, and inappropriate verbal or physical behavior with residents. Students will dress in a professional manner for their clinical experience. Clean, neat scrubs will be worn to clinical. Clean shoes. No facial jewelry. One pair of earrings only. Hair tied back away from face. No extra jewelry. No underwear, thong or otherwise to be visible at any time. Attempts must be made to cover tattoos. (e) No one with a felony conviction or who is listed on a sex offender registry will be admitted to class.

Job Placement

10. (a) Assistance with job placement is available through our clinical sites. (b) Students who request assistance finding jobs in long term care facilities are assisted in filling our applications and arranging interviews. (c) Our clinical sites offer employment opportunities for the majority of our staff.

11. While we do not track individual employment rates we do have facilities willing to offer employment to all of our graduates. Aspen Ridge, Avalon Care Center, Woodland Park Care Center and Sandy Regional Care Center all offer jobs to students upon completion of course work.

Refund Policy

12. The refund policy for both withdrawn and dismissed students is the same.

13. (a) A three-business-day cooling-off period, commencing with the day an enrollment agreement with the applicant is signed or an initial deposit or payment toward tuition and fees of the institution is made, until midnight of the third business day following such date or from the date that the student first visits the institution, whichever is later, during which time the contract may be rescinded and all monies paid refunded. Evidence of personal appearance at the institution or deposit of a written statement of withdrawal for delivery by mail or other means shall be deemed as meeting the terms of the cooling-off period. The $50 registration fee is not refundable after the three day cooling off period but may be applied to a different class. The remaining $350 fee is refundable until after completion of the first day of class for the C.N.A. class. If any part of the phlebotomy class is attended the fee is non refundable. The fee can be applied to a class at a later date. We offer no loans.

15. We offer no loans.

16. Due to the short period of time classes are offered (80 hours and 6 hours) a surety bond or certificate of deposit is not required by the Division of Consumer Protection.

Pass Rates

17. Pass rates for the Utah State Certified Nurse exam for the past 24 MONTHS are cumulative 89% for written test and 88% for skills test. This is +7 and +6 percent higher than State average.
